The CLCREE intake form is designed to centralize your homeowner information so we can review and make the necessary recommendation concerning your predatory lending claim. The form has a number of questions that will give us a clearer understanding on how the mortgage transaction was originated and what real estate professionals were involved in your mortgage (s).
There is no fee to take part in this legal study. There is no guarantee of positive results in your favor.
At present, CLCREE is a non-profit organization with limited resources. You may benefit significantly by filling in as much of the information as possible on your form. The more information you provide, the better we can assist you. Also, the more information you provide, the greater we have available to prove that you may be a victim of predatory lending and track trends of others possibly “taken” by similar organizations and circumstances.
By filling out and submitting this form, you are giving CLCREE permission to evaluate this information and contact you concerning alleged predatory lending practices. Your completed form will be shared with law firms working in tandem with the CLCREE as well as state and federal organizations to initiate legal action depending upon the findings by CLCREE. WHY WE COLLECT THIS INFORMATION
PRIVACY POLICY
At The Center for Litigation & Consumer Real Estate Education (CLCREE) we recognize that the privacy of your personal information and experiences is important.
The information collected on this site is in a secured format and uses Secure Sockets Layer (SSL) online technology, which encrypts the data you provide. You will know you are in a secure area when you see the address bar at the top of your window indicates "https" as opposed to "http" as seen on non-secure websites.
The data you provide is only shared with the attorneys and the legal system we work with to get your case heard. We will notify you via email as to which attorneys and law firms we are working with. If you have any objections, for any reason, we will not share your information.
We ask that you notify us within 10 days of receiving any communication your find objectionable. (If we receive your notification after 10 days we will do everything to retrieve your information and not share it with any law firm). It is that simple!
We recognize that you have been through an ordeal with the real estate and mortgage system and you may have trepidation in sharing any information. We respect this and thank you for trusting us.
The information collected is for residents of the United States. However, at the present time, due to funding and resources, we are concentrating our efforts in the State of California. As funding, and legal services permit, we will expand our services to include the entire United States. If you are from outside the State of California, we will be happy to collect your information and real estate experience. The more information we receive, the easier it is to gain interest in your case through funding and legal support. This organization’s services are not intended for use by persons in other countries or territories outside of the United States.
BOTTOM LINE: We will never share or sell any personal information that you provide on this intake form without your written consent.

GENERAL INSTRUCTIONS
FOR CLCREE LEGAL ASSISTANCE FORM
HANDWRITTEN FORMS: If you do not have a computer, you are welcome to hand print all information, however, it may take additional time to transcribe your information into our secure data base. Please be sure to print clearly if this is your only option. Please be sure to make a photo copy for your records and send the original form to us. If you need more room in the MY STORY section, please add as much additional paper as you need. We recognize that not everyone has access to a computer and we will do our best to accommodate you.
IF ENGLISH IS NOT YOUR FIRST LANGUAGE: Please ask a friend or family member to assist you. We can also suggest that another non-profit organization assist you in filling out this form.
THE “YOUR STORY” SECTION: Please note that when you make a copy of the completed form on your printer, you will not see the entire story you have written. Please be assured, however, that CLCREE will receive all the information when you click on “finalize form.” To have a complete copy of “your story,” copy & paste all information on this section on a word processing form on your computer and save it in a safe place for your records. STEP-BY-STEP
INSTRUCTIONS FOR FORM
NAME SECTION: Include your full name (no nick names please) and middle name. If your name has changed since the mortgage transaction, please include this information in the YOUR STORY section at the bottom of the form.
ADDRESS: Include your present mailing address. If you have moved out of the home that was foreclosed upon, please do not put that address here. This section is so we can contact you should we need to send you certified mail. Please notify CLCREE by clicking on the EMAIL US button at the bottom of this page should you have changes in contact information.
PHONE NUMBER: The best number to reach you in case we have a question or need to notify you immediately. If you have changes in your telephone number after filling out this form, please notify us by email.
EMAIL: At CLCREE, we use email as our primary way to communicate with you. This method saves money for CLCREE in postage, time, human resources, etc. We recognize that not everyone has access to a computer and we will do our best to accommodate you. If you do not have your own email address, you can get a free email account from the following websites: www.Yahoo.com; www.GMail.com; or www.HotMail.com.
PROPERTY ADDRESS SECTION: The address of the property for which you had a mortgage(s), or tried to get a loan modification.
PROPERTY PARCEL NUMBER: This is the property identification number, also called assessor parcel number that you can get from your county tax assessor. This is an extremely important number for CLCREE and may take some research. You can find the property parcel number on your real estate tax bill, deed, or appraisal. If you do not have any of this information, telephone the county’s assessor’s office and give them the property address and they can provide you with the property parcel number for free.
MORTGAGE COMPANY SECTION: The name of the mortgage company responsible for your loan. Please provide this even if the company is no longer in business.
BANK SECTION: Your bank and mortgage company may be the same thing. If this is the case, please type “NA” for non-applicable. On some computers, the form will not be sent to CLCREE unless every section of the form is filled in.
APPRAISER SECTION: The name of the real estate appraiser. This can be found on your appraisal report. If you do not have an appraisal report, order a copy from the bank. If the bank will not give you a copy, contact CLCREE by email. The appraiser cannot provide you with a copy, however Federal law requires you to receive a copy from your bank or lender.
REAL ESTATE AGENT SECTION: If it has been sometime since you purchased your home, this information may be inapplicable, especially if you have recently re-financed your home and a real estate agent was not involved. If you feel your case involves a real estate agent, it is important that you provide as much information as possible.
DATE OF LOAN: The date you signed the loan. If you have more than one loan please note in the Loan Details information box. IMPORTANT INSTRUCTIONS IF YOU FILL OUT THE LOAN DETAILS SECTION: As stated, above, please note that when you make a copy of the completed form on your printer, you will not see the entire story you have written. Please be assured, however, that CLCREE will receive all the information when you click on “finalize form.” To have a complete copy of “your story,” copy & paste all information on this section on a word processing form on your computer and save it in a safe place for your records.
LOAN ID: Loan identification number. If you have more than one loan please note in the Loan Details information box.
YOUR STORY SECTION: Please give CLCREE a detailed narrative on what happen to you concerning the Mortgage and/or Loan Modification so we can take your words and be your voice in the future possible litigation action.
ADDITIONAL INSTRUCTIONS: We recognize that this is a very detailed form and it may not be possible to provide all this information in “one sitting.” If you are able to only fill out parts of this form at a time, click on the SAVE FORM button at the end of the form.
